Not sure if you are supported or not? If you are receiving a monthly or
bi-monthly paycheck from MTU, then you are most likely supported. Contact
student_insurance AT mtu.edu for help with determining your
support status.
Frequently Asked Questions
- How do I get the insurance to pay for my
medical costs?
You should provide your ID card and/or your insurance information to your doctor's office or healthcare provider so they may bill the company directly. - How do I get an ID card?
You should receive an ID card via regular mail to the address you provided on your insurance forms. You may always print out additional ID cards from the Student Insurance website. - Do I have to see specific Doctors in
order to use my insurance?
Yes - sort of. In order for covered expenses to be paid at ~80% of Usual and Customary charges after your deductible, you must see a participating network physician or healthcare provider. MTU graduate student health insurance participates in two networks. Lists of participating providers are available at the network websites, www.beechstreet.com and www.ppom.com. Covered expenses incurred through visits to out-of-network providers will be paid at 60% of U & C.

- The health insurance subsidy is $1,150 per supported student per year for 2005-06 term.
- For the supported student plan, the total cost for the
individual student's plan is $276.
The entirety of this will be deducted from the student's payroll in the fall semester.
